Roughly 150 yards uphill to the west of the Lower Adit, the Upper Adit of Lady Slad Freemine is barely open. The entrance is small—around 1 foot high and 4 feet wide—and partly blocked by collapsed material, requiring some clearing for entry. To reach it, continue uphill from the Lower Adit, crossing a forest road that follows the 575-foot contour. Look for a sharp hairpin bend; just beyond that, about 50 feet higher, a fence marks the location of the adit.

The area around the Upper Adit contains signs of older and possibly separate workings. Just south lies a filled-in shaft that appears to predate the adit, while 25 yards to the north is a depression that could be either a collapsed shaft or an adit. Continuing another 100 yards uphill brings you to a filled-in shaft, now a circular depression about 15 feet across.

Approximately 300 yards to the northwest is a large spoil heap at SO 5976 1300 (700 ft elevation), heavily disturbed by forestry operations. Though overgrown, this significant mound—about 10–15 feet high and 100 feet across—indicates the presence of another former shaft, though its exact location is now obscured.
